Please note, that a current and physician-signed sports physical must be on file with Proctor High School before participation (this includes the first day of practice). Sports physicals are generally good for three years from the date of examination based on your physician's medical examination. TIP: Scheduling your student's sports physical between July 15 - August 15 (the start of MSHSL high school fall sports) will ensure your student is prepared for the first day of practice, and it will carry your student through a full three years of school sports. Sports physicals must be turned in to Mr. Anthony Wood at St. Luke's Sports and Event Center or you may email it directly to him at awood@proctor.k12.mn.us before the first day of practice.
Further questions may be sent to Anthony Wood, Athletic Director at awood@proctor.k12.mn.us
